Each child is paired with a dedicated Care Manager who works with the family to create a care plan built around the child’s unique strengths, challenges, and goals. These plans are personalized and designed to grow with the child over time.

Care Managers connect families with healthcare, mental health, education, and community services. They act as the central point of contact between schools, doctors, therapists, and service providers—ensuring everyone is working together.

We help families navigate complex systems such as Medicaid, C-YES, mental health programs, and IEPs. Our team makes sure families understand their options, access available services, and get the support they’re entitled to.

Care Managers stay in close contact with families—checking in regularly, attending care team meetings, and making sure services remain in place. This continued guidance helps families feel supported and empowers them to make informed decisions.
HCBS stands for Home and Community-Based Services—a life-changing program under the NYS Department of Health. It’s designed to help kids and teens (ages 0–21) with medical or mental health challenges that put them at risk of hospitalization or out-of-home placement.
