What Is HCBS?
Home and Community-Based Services (HCBS) provide individualized, hands-on support for youth who are at risk due to medical or behavioral health needs. Services are delivered one-on-one in the youth’s natural environments—such as the home or community—and focus on meaningful engagement, supportive supervision, the development of life skills, and promoting independence and resilience. Referrals are made through the youth’s Care Manager as part of the New York State Children’s Waiver program, ensuring that services are coordinated within the youth’s overall plan of care.

Our Prevocational Services help youth (14+) discover career interests, gain workplace readiness skills, and develop confidence through hands-on learning and volunteer experiences. Providing hands-on coaching for participants and their families
Caregiver & Family Support Services (CFASS) provide hands-on coaching and practical training designed to strengthen daily life at home and in the community. We work directly with participants and caregivers to build skills, confidence, and routines that support independence, connection, and long-term success.
Respite Care

Caregiving is one of life’s greatest responsibilities, and it’s okay to need a break. Our Respite Care program provides dependable short-term relief for caregivers while ensuring youth continue to learn, explore, and build meaningful connections.
Palliative Care Counseling and Support
Delivered with empathy, respect and thoughtful care
This service provides compassionate counseling for individuals and families facing chronic or terminal illness, focusing on emotional well-being and navigating life’s challenges with dignity. Services are available in the home or in a clinical setting.
